Is there a fee to use MyChart?
MyChart is a free service offered to our patients.
How do I sign up?
Patients who wish to participate will be issued a MyChart activation code during their clinic visit. This code will enable you to login and create your own user ID and password.
Parents/Guardians of patients 15 and under can also set-up a MyChart account to access their child’s information. To establish this access, parents must complete and submit a Proxy Consent Form. Proxy Consent Forms can be obtained from any of our pediatric offices.
Can I schedule an appointment with my doctor through MyChart?
Yes, MyChart allows you to schedule an appointment with an Our Lady of the Lake Physician Group primary care provider that you have already seen before.
When can I see my test results in MyChart?
Your test results are released to your MyChart account after your physician has reviewed them or within 3 days.
Why are certain test results not shared electronically via MyChart?
Your provider determines which types of test results are able to be accessed through MyChart. Test results of a very sensitive nature are not released to MyChart.
If some of my health information displayed on MyChart is not correct, what should I do?
Your MyChart information comes directly from your electronic health record at your doctor's office. You can message the clinic with the correction or ask your doctor to correct any inaccurate information at your next visit. Your health information is reviewed and updated in your electronic health record each visit.
If I send a message to my doctor or nurse, when can I expect a reply?
Your message will be reviewed during regular office hours and you should expect a response from the clinic within 1-2 business days. Please note that MyChart should not be used for urgent situations. Please contact your provider’s office directly if the situation requires immediate attention or dial 911 if it is an emergency.
Can I view a family member's health record in MyChart?
Yes, this is called Proxy access and allows a designee access to MyChart information regarding their family member.
Proxy access can be established by family members for patients of all ages. Patients over the age of 15 will need to consent to their family member’s proxy access request for their account. Parents or guardians of patients 15 and under can simply request a MyChart account for their child to access their information.
To activate proxy access, you will need to complete a Proxy Consent Form which can be obtained from any of our offices. Please return the completed from to OLOLPG –Customer Service Department, 8415 Goodwood Boulevard, Baton Rouge, LA 70806 to request access to this convenient service.
Can I ask questions regarding a family member from my MyChart account?
Unless you have proxy access activated for your family member’s account, you will only be able to access your MyChart account. Since MyChart offers direct access to your personal health record, sending a question about a family member would result in the question about their health to appear in your chart.
Can my spouse and I share one MyChart account?
No, due to the sensitive nature of medical information and because each MyChart account is directly linked to your health record, each adult must establish their own MyChart account.
I forgot my password. What should I do?
You may contact our MyChart Patient Support Line at 225-765-5727 or firstname.lastname@example.org to request a new, secure password. You may also click the "Forgot password" link on the sign-in page to reset your password online.
Can you send me a new activation code if I have lost it, let it expire or did not receive it?
Contact us at email@example.com and after we verify your information, a new code will be sent via U.S. Postal Mail. Privacy issues prevent us from e-mailing a new activation code to you.
Where can I update my personal information (e.g., home address, e-mail or change my password)?
Log into MyChart and from the left menu, go to the Preferences section and select the appropriate option.
How is MyChart secure?
We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal ID's, and passwords. Each MyChart user controls their individual password and the account cannot be accessed without that password. Further, MyChart uses the latest encryption technology to automatically encrypt your session with MyChart. Unlike conventional e-mail, all MyChart messaging is done while you are securely logged on to our website.
MyChart is fully compliant with federal and state laws pertaining to your privacy. Your name and e-mail address will be treated with the same care and privacy given your health records and will never be sold or leased by MyChart.
I was logged out of MyChart, what happened?
We aim to protect your privacy and security of your information. While logged into MyChart, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of MyChart. We recommend that you log out of your MyChart session if you need to leave your computer for even a short period of time.
Is my activation code my user ID?
No, your activation code is not your MyChart ID or password. You will use this code only once to log into MyChart for the first time. (The code will expire after you have used it or after 30 days). When you log into MyChart the first time, you will then be asked to create your own unique MyChart ID and password.
My activation code does not work. What should I do?
If your activation code doesn’t work, email us at firstname.lastname@example.org or you can call our MyChart Patient Support Line at 225-765-5727.
Please Note: MyChart may not be available in all Our Lady of the Lake Physician Group locations at this time.
