Phone Requests
Routine telephone questions and requests for medication or prescription refills are only taken during regular office hours. To obtain test results, please ask for your nurse's voice mail and leave a message. He/she will get back with you as soon as possible.
PRESCRIPTIONS
Please have your pharmacist call or fax your physician's office with your requests. Refills will be called in for enough medication to last until the next available appointment. Prescriptions for narcotics will not be refilled without a visit to your physician.
EMERGENCIES
In a life or limb threatening emergency, please go to the nearest emergency room for assistance or call 911. When treated in an emergency room, the patient is responsible for meeting all insurance company requirements. For all other urgent care and/or after hours needs, call your physician's office. You can also call LakeLine Direct at 765-LAKE (5253) or toll free 1-877-765-LAKE (5253) for free 24-hour nurse advice and health information.
REQUESTS FOR MEDICAL RECORDS
Obtaining a copy of your or your child's medical record requires a signed Authorization for Release of Protected Health Information form. Copying records is done on a weekly basis by a company hired to process these requests. You will be billed for this service directly by the company. Once your payment is received the records are released via mail to recipient you've indicated through your original request. Fees for this service are based on Louisiana guidelines.
REQUESTS FOR IMMUNIZATION RECORDS
To receive a copy of your child's immunization record simply call your physician's office and request one. Please allow 48 - 72 hours to process your request. Records can either be mailed directly to you or picked up at the physician's office but we are not able to send records via fax.
