What are the office hours of a particular doctor's office?

• Our Lady of the Lake Physician Group consists on nearly 40 clinic locations and each clinic sets its own office hours.  Contact information for all providers and clinics is easily accessible using our physician directory. 

How can I obtain a copy of my or my child's medical records?

• Obtaining a copy of your or your child's medical record requires that you sign an Authorization for Release of Protected Health Information form.  This signed form submitted at your current or new physician's office.  Copying records is done on a weekly basis by a company hired to process these requests.  You will be billed for this service directly by the company.  Once your payment is received the records are released via mail to recipient you've indicated through your original request.  Fees for this service are based on Louisiana guidelines.

How can I get a copy of my child's immunization/shot record?

• To receive a copy of your child's immunization record simply call your physician's office directly and request one.  Please allow 48 - 72 hours to process your request.  Records can either be mailed directly to you or picked up at the physician's office but we are not able to send records via fax.

What insurances are accepted by Our Lady of the Lake Physician Group?

• Our Lady of the Lake Physician Group providers accepts payment from most verified insurance carriers.
